Our team can take referral details over the phone during opening hours, and will confirm receipt of every referral sent through this form.
A valid referral includes
- Referrer name, practice and provider details
- Patient name, date of birth and contact
- Reason for referral and relevant history
- Any recent x-rays or imaging, attached

If your patient has already been referred, you don't need to complete the referral form again. Send their CBCT or other imaging straight through here.
Secure CBCT Upload
CBCT scans are uploaded separately via our secure ShareFile portal below. You'll be asked to enter some details again — this helps our team match the scan to your referral.
Please upload your CBCT scan as a single .zip file only. Do not upload individual DICOM files — zip the entire scan folder first.
The CBCT file may take up to 2 minutes to upload. Please wait until the file has been completely uploaded before submitting the referral form.
